Please confirm the person trying to register was the Customer Contact (Sold-To-Contact) on an existing sales order.
iStore 'Need Online Access Registration for B2B' validates and then creates the username for the contact mentioned in 'Customer Contact' (Sold_to_Contact).
Hence, this information has to be provided when an order is placed.
You can use the following query to check -
from apps.oe_order_headers_all where order_number=&order_number;
Run scripts to check values are numeric from document -

The "Registration for User of an Existing Company" is for a user that has a relationship (ie Contact Of or Employee Of) with a company (Organization).
