In Reports--> Data Model --> User Parameters add a parameter 'p_user' then assign the value from forms. I think you have some 'username' which might be globally defined, Pass that Global variable to the P_user in your forms (Program_units--> report(Procedure)). It will work for sure.
Then For your next question, Oracle saves the number format of '012343' as '12343'. If you want to show that '0' means you have to change the datatype of the column as VARCHAR2.
