I have resolved this problem by removing this entry from the server.xml:
<jvm-options> -server -Xss128k </jvm-options>
Unfortunately, I do not know why this should resolve my problem, but I am certainly happy that it has.
The -server shouldn't cause you problem.
The -Xss is probably the culprit since it defines the maximum native stack size for any thread to be used by your Java web apps. You are better off without it anyway unless you have other native plugins or JNI that need to be 'controlled' in which case you would need to try with different values like -Xss256k or -Xss512k.
