Custom customer statement form

Hello,

How can we attach a specific custom customer statement form to an email template.

We would like to be able to use the "include statement' checkbox when emailing from customer record, or to somehow attach a statement to email template but with a specific custom form.

Is that possible in NetSuite? Through Workflow/Script?
