Extension Manager - unable to install

Anyone else have problems getting the Extension Manager to install and the "Manage Extensions" menu item to appear under SCA?   Can SCA be installed with SiteBuilder on the account?  I think that is what is causing all the problems but can't be sure.

Been working with support on and off all day for the past 7 hours and I am at the same place I was 7 hours ago.
