SuiteScript 2.0 entry point scripts must implement one script type function

Hi,

I'm new to the suitescript process. I am trying to create a dynamic form to enter a new customer and keep getting the error in the topic name. I am not sure how to fix this as there isn't any info I can find to address that error.

The code for this script is:
