Where is the "Notify Sales Rep by Email" Form Template?

I want to customize the email a sales rep receives after an online lead form is submitted but I can’t seem to find it. I can find all the other templates but not this one.

Ideally I’d like some of the information entered in the form to populate the email sent to the sales rep. Does anyone now where this template may be hiding and if it can be customized? I’ve looked everywhere!
