Entry Form as a Search Form

Hi all,

Is there a way you can perhaps use an Entry Form as a specific 'search form' whereby it does not allow the entry or creation of a new record, but rather, searches the database and provides a list of the search results?

Specifically, what I'm attempting to achieve is to create a search for our Partners, where they can enter a potential prospect's Name (mandatory) and at least one of the following (also mandatory - this is one of the reasons why simply using a Saved Search and Filters does not work): Phone Number, Email Address, and / or Company Name, and to then provide a very limited response back to them essentially indicating if that person exists in our database or not.  I have mocked up an Entry Form, and created a Custom Actions "Search" button, but for some reason I
