Invoice address not propagated and other errors

We are finding that random sales orders are not propagating customers address.

Worse than that, earlier today I had entered a sales order for a customer but did not complete it because the customer declined to go ahead. Instead of pressing the "Reset" button I overtyped the customer to the next customer and it refreshed the delivery address but left the invoice address set at the details of the previous customer.

These actions are not reproducible, they seem to happen randomly.
