missing add/insert/remove/cancel buttons on line items

Hi all, is anyone else having trouble on various browsers (firefox/ie) with missing add/insert/remove/cancel buttons on line items when they load up a transaction form? 

This seems to happen intermittently in the last 4 days.  In some cases a cookie/cache clear and reload helps, in some cases it doesn't.  Also loading system2.netsuite.com can help as well.

There is usually a javascript error of some sort displayed for the IE browsers, when it next happens I'll update the thread here. 

But I'm trying to isolate if its something anyone else is experiencing (given it only started this week and we haven't added any scripts/customizations in that time) or if its only us.
