WSDL v2.6 / File Cabinet / File / FileAttachFrom

Has anyone successfully uploaded a file cabinet item using fileCabinet.xsd

Specifically, I am struggling with:
attachFrom
and
FileAttachementFrom

There is something I am missing. Unfortunately, there is no documentation -- yet.
