Show us Your Clever Uses of Online Customer / Case Forms

What ways have you found to make clever use of online customer forms or case forms?

We have a variety of forms hosted outisde of NS that we are migrating to NS. So far, we have managed to get a catalog request (online customer form), contact / inquiry (support case form), and a form for folks to suggest new dealer locations (support case form). 

I'd like to add some more, specifically one for folks to give ratings to our biz in several areas, and maybe one where folks could do item reviews. The problem is, Netsuite doesn't seem to have much pre-made functionality in this are ( I would love to see some pre-loaded form templates for coming things) so I'm turning to the community for inspiration.
