Reference Cart - Shipping Estimator 'broken' by NetSuite Gift Certificate Shipping Fix

I am installing Reference Cart 2.05 on a clients site. All is going well as it's about the 20th time I've installed the Reference Checkout on a SiteBuilder site.

They need to have Gift Certificates be allowed to be ordered by themselves so we had to apply the patch NetSuite specifies in SuiteAnswer 46484  which was last updated on 01/28/2016

However, when I apply this patch, the shipping estimator in the shopping cart "breaks": meaning you can add a zip code, press the estimate button, the ajax-loader.gif displays for a few seconds, then it stops displaying and nothing happens. No shipping estimate.
