Update customer record form

Ok I want to create a form that has a url i can send via an email campaign which allows a user to update their customer record, I would like to send the form to pre-populate with the ID and their name but allow all other fields to be updateable.

I have tried a few dummy forms but they seem to fail on the updating of an existing record and simply create a new one, any pointers on this??
