Sample Headers for CSV Pricing Import

I need to do an item import to update several items. Each item has between 30-40 different pricing (5 quantity breaks, 6 different price levels). I figured I am going to have to break the file down to 3-4 different CSV files (each containing 2 price levels with 5 pricing breaks). What I am having trouble with is the headers for the CSV imports. I am including the Internal ID for the item in column A for each file to link them. Please help!
