Alert Window in Checkbox is False?

We're encountering an issue where I need to generate a popup alert window if a checkbox is not checked.  The checkbox is a custom field (custbody_release_to_ship), and this would only apply to the Item Fulfillment screen.  I haven't done any javascript coding in years, so I'm running into a bit of a wall here.
