Save and Execute missing

In a script with 4 deployments, the "Save and Execute" choice is missing from the most recently executed deployment. That would indicate the script is still running, but there are no entries in the script status page for any of the deployments. If a different deployment is executed it will end up in the "Deferred" status and will not run. There are no records in the execution logs.

How can the "Save and Execute" be restored? What is likely the underlying problem?
