Set Custom Transaction Column Field Mandatory
I need some major help.. I know I have some right pieces in here, but I also know I just guessed/made some other stuff up! I bolded what I made up.

Tentatively I have the type = create in there, but I would prefer it to be if the custom form = a certain one... any ideas on how to do that? I have tried customform = 148 and entityid= 148, but neither seemed to have worked. I am new to this! Go easy!
