attachSolutionToCase not defined on Custom Case Form

Hi,

I have a Custom Case Form which has the normal controls for inserting a Solution into the outgoing message (i.e. outgoingmessage field) for the case.

When I try to actually insert a Solution on the "Standard Case Form" using these controls, it works normally as expected.  When I use my Custom Case Form, nothing appears in the outgoingmessage field and my Javascript console gives me the error:

attachSolutionToCase is not defined

In the Standard Case Form (ID -100), the function attachSolutionToCase() is called from and appears in the JS script file at "https://system.netsuite.com/app/crm/support/supportcase.nl?id=28553&e=T&cf=-100
