Form for Printing Deposit Slips

I am looking for a deposit slip form that can be used with NetSuite (similar to check vouchers) to laser print bank deposit slips. (Transactions> Bank> Make Deposits > List, and click Print next to the deposit slip you want to print to see the PDF.)

I could not find guidance in the online help or knowledge base.  I did see references to forms from NEBS for printing checks.  I was informed by a NetSuite sales rep that www.nebs.com supplies deposit slip forms (as well as check vouchers) that can be used with NetSuite to print deposit slips.
