How to display only unique records

Hello,

I am trying to create a report for our Support Management team. This report is showing a Customer Survey results which are stored in our Custom Record. This Custom Record is also storing an Internal ID of Support Case which it belongs to. Sometimes it happens that there are 2 existing (ussualy duplicated) Custom Records for the same Support Case.

Report should only display one of those duplicates. I found some advices it could be done by DISTINCT function, unfotunately I wasn't able to make it work.

Could someone help me with this?
