Name, City and State not going to PayFlow

We recently started processing credit cards with PayFlow gateway through NetSuite.  We process the orders manually.  It works great but the Name on the Card, City and State do not get passed to PayFlow.  We are entering this information but it does not make it through.  All other information goes through.  PayFlow is processing the cards like this but they have requested that we get this resolved. 

I don't see any other posts or knowledge base articles on controlling what information gets passed.  Anyone have ideas?
