Shortcuts aren't sticking

Some colleagues of mine have shortcuts set up that don't stick in their shortcuts menu. Every time they log on, they have to click 'set up shortcuts' and save to get them onto their shortcuts list. Has anybody seen this before? I thought the issue was role based, but it is not. Any thoughts/ideas would be greatly appreciated!
