Problem with Advanced PDF/HTML Packing Slip...won't print 'Ordered', 'Backordered', and 'Shipped'...

We seem to be having a problem with the Advanced PDF/HTML Packing Slip printing the expected data.

We don't seem to have the 'Ordered', Back Ordered', and 'Shipped' columns printing with data.

Instead, there is only a 'Quantity' column printing which just shows all quantity that was ordered on a sales order.

We have not made very many changes at all to the 'out-of-the-box' Packing Slip form (just our own logo/footer/some additional text to print in the body of the Packing Slip.

The following is in the Source Code of the area of the Packing Slip that I believe determines the labels and the data that should print under the columns I'm asking about.
