I cannot exclude the department or class from the Build Assembly form no matter how I try and custom

No matter what I try I cannot customize the Build Assembly form to eliminate the class and department fields on the sublist. I can only remove it from the main body of the screen. I've tried it by customizing the form (no go) and from in edit mode customizing the form where you check and uncheck additional fields (not even there).
