PDF Template with Form Fields not Showing Data in BIP

I've created a BI Publisher Data Model and Report and I'm trying to upload a PDF template that has form fields with the XML columns inserted. When I run the report they are showing as blank/not pulling data in from the data model. I've attached the xml, Data Model, Report, and the PDF I'm using. Has anyone had experience with this or is there a format that needs to be used? I'm following the guide below.
