Running an OTBI analysis report from a new tab attached to a form using Forms composer

Question

Is there any documentation or advise on how to configure (using page composer) a new tab on a form that when clicked opens up a previously configured OTBi analysis report that contains one parameter to be populated by the user?

I have attempted the steps in the attached doc but nothing is working so clearly my configuration is not complete and there is also very limited documentation available that i can find that clearly shows the steps to achieve my objective.
