Custom report to monitor login activity

Leadership would like a custom report to monitor login activity.

We'd like to know which users logged in last week and which application/function they used.

Sample output:
UserID, Login Date (is there a time of day available), Application/Function
12345, 19-Jun-2024, DoR
12345, 19-Jun-2024, Personal Details
45678, 20-Jun-2024, Payment Methods
45678, 20-Jun-2024, Contact Info
45678, 20-Jun-2024, My Public Info

Not sure if this is available in BI Subjects or custom SQL. Happy to receive guidance with either. Appreciate your help with this.
