I've had this same issue on numerous installs. I once spent a week or so digging into fixing it, without resolution. Now I just ignore it because I can do each step in BIACM directly. On deadline, I've always just used FSM to get the check list tasks, and then go directly back to BIACM to do each step. The help (?) icon in BIACM will tell you the instructions for each step, which are the same directions/instructions you would get from FSM. Other places to look for instructions are the two documents from the docs.oracle.com page for your OBIA version called Functional Configuration and Configuration Guide. My recommendation would be to open an SR and in the meantime, feel free to start the tasks directly in BIACM. You can search support.oracle.com for the exact error because there are several SRs and Bugs for this and OBIA 11x versions, but I would recommend caution when trying to implement those steps, as versions are different and your mileage may vary.
