Create Customer Account via SOAP web service

I am trying to create a customer account via the soap web service. I have been looking at the following document to help

WebService Sample Payload for Creating a Customer Account (Doc ID 1334993.1)

It has proven to be very useful but I am unable to add CustomerAccountContact details as part of the request as I do not have a valid relationship Id to pass in.

How do I create the relationship between account and contact so I can use the relationship ID to create the CustomerAccountContact?
