Is there an OEM 12c that is not cloud control?

Yes, to both. It still exists as DB standalone -- https://docs.oracle.com/en/database/oracle/oracle-database/19/admqs/getting-started-with-database-administration.html#GU…
And Cloud Control (to manage multiple databases at one time) is also included in the DB license. Based on the diag/tuning packs that licensed, additional features can be used.

Not sure i get the question... The product name is Oracle Enterprise Manager 12c Cloud Control, but it's for on premises or cloud. There's also Oracle Enterprise Manager Database Express - which is a standalone OEM for one database. This is run locally in the database itself.

Many year ago it used to be know as dbconsole or just OEM and was located on the database server.
It came with the installation of the database,
It was just for monitoring one database.
Now there is OEM Cloud Control and you can monitor multiple database, asm targets, application servers, etc.
I am looking to see if there is OEM for monitoring one database and comes with the license for the database.

Is there Documentation for 12c?

I have a couple of more questions please answer or send me a documentation on where I can find it.
1. In Database Express can I run data guard admination and execute switch over and fail overs?
2. Can I manage my database agents in Database Express?
3. Can I manage automatic storage management (ASM) in Database Express?
4. Is there an express for Middleware, weblogic, forms, reports, and agents?

I'm more familiar w/ Cloud Control, so not sure about Data Guard or ASM. I would probably lean towards Cloud Control at that point.
FMW Control is the equivalent for middleware side.
If you have more than 1 db and fmw, i would suggest looking at Cloud Control as it will allow you do utilize mroe features. Licensing is the same whether you use Cloud Control or Database Express. If you have EE with Diag/Tuning, there's a lot available to you. Repository DB install + mw used for Cloud Control is available at no cost as part of the limited use licensing.
