we encounter this issue with an SOA instance

this was an issue of iscsi-initiator-utils.x86_64 0:6.2.0.873-27.0.3.el6_9, upgraded to iscsi-initiator-utils.x86_64 0:6.2.0.873-27.0.11.el6_10 and then the instance was able to start.
