'Select List' Application Item displays Null instead of 'Select a Value'

I have defined an application item as a Select List and I am trying to display "Select a Value" for a null value. I have set 'Null Display Value' to 'Select a Value' and set Display Null Values to Yes. I would like the message "Select a Value' to always display when the user is entering initial values to 'CREATE' an entry in the table. The user is not required to enter a value. I just would like to display the message anyway. How do I 'make' APEX display the message. Even though I have set values, nothing displays.
