Redirect to Pages using SelectList

I have Created an Application with 10 Pages. and also i have a SelectList item in 4 Pages. If iam changing selectlist to the second LOV it should redirect to Another page in the Application, also if am Selecting the third LOV it should redirect to another Page. What should i do for this. Give me any solution.

First i need a Button Name:"Add Comment" that should in the last column of the Report. How to Create a Button inside the Report.
