Take another look at your own screenshot. The file size is 0. Likely you have damaged the file or it somehow has been corrupted.

.1. Exactly which Forms version are you using?
.2. Is your Windows user account the same user account used to perform the Forms installation?
.3. Have you confirmed that the file you are trying to open exists in the directory you believe it is in?
.4. It looks like you are trying to open "webutil.olb". Did you copy this file from another installation OR are you trying to open the file that comes with the installation?
