If the image file is on the middle tier you can use the built-in READ_IMAGE_FILE to load the image file into a Forms "image item". If you want to do similar on the user-tier you would need to create your own Java Bean or WebUtil enable your form and use CLIENT_READ_IMAGE_FILE.

If you are wanting to read directly from an image scanner and stream the result into the form, you would need to create a Forms Java Bean that can do the work. If you are not familiar with Forms Java Beans, there are many examples on the Internet. You can also find some information in the Form Builder Help.
