Oracle APEX Form Region Not Saving Data

Hello,
I have a form region in my application. I am not sure what happened, but it does not save data. I got it to work before, but for some reason it is not anymore. I am on Apex 21.2.
It is using a Automatic Row Processing process.
I uploaded it to Apex.oracle.com if anyone wants to take a look.

I get a record was saved message after trying to save, but nothing is saved in the end. Thanks.
