I'm also experiencing the same issue, I'm running Glassfish Opensource edition
on Windows XP, Oracle XE and APEX 4.0.
Any help would be appreciated.
I had the same problem and I downloaded OracleSupportFiles.zip from Oracle. It contains xmlparserv2.jar which works with FOP.

I'm getting the same error.
Glassfish 3.0.1 with APEX 4.0.1 using the apex_fop.jar supplied with apex.
According to my database oracle.xml.parser.v2 is installed, valid and enabled.
Am I missing a configuration setting?
Could you provide the URL you downloaded from?
Also excuse I'm not too familar with deploying jar's, where do I need to deploy this to? The db or application server?
Here is what I did to get it to work.
1. Copy xmlparserv2.jar from my ORACLE_HOME/oc4j/lib directory to my glassfish domains/domain1/applications/fop/WEB-INF/lib directory.
2. Added the following two lines to the glassfish domains/domain1/fop/apex_fop.jsp before the driver.run() line
The second part of solution I found on another forum page.
