there are other possibilities, but failing/failed disk sounds probably the first check - also iostat -nE should point to hardware/transaction based errors
This error will occur is the disk is dead, dying or has bad blocks (Assuming there are no connectivity/bus issues).
If the disk is dead or dying, replace the disk.
If it is due to bad blocks -- you should see the blocks in the messages file. Move the data off to another subdisk, if possible. Otherwise back up the data, unmount, stop the volume, delete the volume and associated plexes and sd, mark the block/blocks as bad blocks using the format command, recreate the volume and recover from the backups.
