unpivoting table

hi all,

hi have a table sample with following data


EMPNO ENAME JAN FEB MAR APR MAY JUN JULY AUG SEP OCT NOV DEC
1 RAJ 23 32 25 53 34 56 34 64 64 34 74 23
2 RAM 25 37 65 58 33 86 74 74 34 84 73 93


i need to have data in below manner, how can it be done...any help please



EMPNO ENAME MONTH SALES
1 RAJ JAN 23
1 RAJ FEB 32
1 RAJ MAR 25
1 RAJ APR 53
1 RAJ MAY 34
1 RAJ JUN 56
1 RAJ JULY 34
