Open the purchase orders form (make sure you are setup as a buyer)
Enter the required fields - make sure type = blanket purchase agreement
Submit for approval.
Once approved, open the Blanket again and create a new release.

You can create BPA by using
1) Normal PO form or through 2) Buyer's work center (if you are using R12).
Before creating a BPA make sure you defined yourself as a buyer...
For BPA release creation, please use oracle standard release form..

Do you want to create a BPA/releases automatically.. Are you using iProcurment?
You need to play a bit with WF attributes... To create a BPA/releases automatically from a requisition you need to set following attributes as "Yes" in wf.
1) AUTOCREATE_DOC: set this to "Y" if you want BPA/Std PO to be created from PR automatically
2) AUTO_APPROVE_DOC : set this to "Y" if you want BPA/Std PO to be approved which was created from PR.
You can use below query to check the current status of WF attributes...
select rtrim(w.name) name,
'Is Automatic Approval Allowed?' Attribute
from wf_item_attributes w
where w.NAME = 'AUTO_APPROVE_DOC'

The process to create BPA from requisition is a little convoluted.
Go to autocreate screen.
Pull up the requisition.
At the bottom, select Document type = RFQ and create an RFQ.
Open the RFQ.
Tools > copy document > Create std. quote
Then open the quote
tools > Copy document > Create BPA
