You mentioned that the approver try to approve requisition through mail... Is the mail interfce is configured by your DBA in your system? Outlook/Lotus note mail should talk to Oracle in this case... Check with your DBA for details...

I observed one more thing.
User has approved three times over mail. When i saw the records in wf_notifications table , i saw that three sets of notification with cancelled status and one with OPEN status.
That means, i think every time , approver is acting , notifcation is getting cancelled and a new notifcation is getting generated.

Could you please confirm if approval through mail was working in your system before or for the 1st time you are having this issue? Are you sure that "Approval through mail" related setups are in place in your 11i instance??
Yes , it is working fine. Even the same approver was approving the requisitions for long time.
The issue has come recently.
Try to isolate this problem between mail client and EBS.
In the email you have the links to post the response. Clicking on the links will do one of the following.
(A) Open the mail composer by defaulting the required text. The user need to send the mail.
(B) Can open the EBS notification screen page and ask the user to fill the required details.
What is the user experience in this case?
In short: If the response is triggered from the mail client and if it reaches the server, the server should take care of processing the response. If the mail client itself does not trigger the response, then there is nothing much you can expect from EBS side.
