Simply speaking: something is wrong with configuration of your conversion server (IBR). Most likely, images are sent to IBR and never come back. You will need to analyse IBR logs to see what the problem could be.

Access IBR Console - Administration - Agent management - <agent name > - check the Full verbose tracing option .
Now checkin any image file (which is failing) - then go to Administration - Logs - Agent Logs - that will list the entire set of steps being done to reach the point of failure .
Also turn on IBR verbose tracing options - refinery* . This logs will be part of Server Output under Administration - System Audit Information .

I check the Full verbose tracing option, and CHECK IN image file, the log looks normal:
Information, 12-7-17 7:00 pm, the conversion job '585326 'log entry:The steps DigitalGraphicRenditions has been completed.
Information, 12-7-17 at 7:00 pm, the conversion job '585326 'log entry:Successful.
Then I look at the Content Server log, found an error and three warnings:
Errors. 12-7-17 11:55 AM. Error in the background processing conversion. Can not find parameter 'dRevLabel'.
Warning, 12-7-17 10:42 AM can not determine the role of guest privileges. The group Adminsitrators could not be found. The exception error type 'java.lang.Throwable'. [Details]Content Server has generated a diagnostic warning. The following stack trace can display detailed information.
Another two warnings, the difference is that the guest replaced by admin, sysmanager.
