the reason for that error is that the tabular form doesn't have a MRU process, but the meta information of that process is required for re-displaying the tabular in case of an error.
I have added the missing MRU process with a condition of type "Never" which will give APEX all the necessary information for re-displaying the invalid tabular form data, but because of the "Never" condition it will not fire in case of a successful submit.
I will inform the developer in charge if it's possible to improve the error message for 4.2 if the MRU is missing.

Too bad, I wasn't able to reproduce it on apex.oracle.com
No idea what could be causing this error, and the entire tabular form disappears when the user-defined exception is displayed.
I even tried deleting the tabular form and created it again, but still getting that error.
I have been battling the same or similar problem for past 3 weeks. In my case it triggers if you add say 5 extra rows and on the 5th entry you leave the row incomplete (hence fails validation) and I get a report error. if you click submit again then it I get the User Defined Exception

And then if I hit submit, again then get the user defined exception
I get something like this in the debug right before the user-defined exception

But in my case it's not the adding of extra rows, that's causing this because I don't use that for my tabular form.
I wish I could help, don't know what's up with this error.
