Use tcRequestOperationsIntf API to provide Request Comment... It can be obtained i.e. read and sent via email too later...
addRequestComment(long plRequestKey, java.lang.String psComment, java.lang.String psCommentType)
Adds a user comment to a Request.
use ootb comments or create your own attribute to store Rejection comment. if required mark it mandatory.
While sending mails read the same attribute

but how do i add it in approve/deny/re-assign form.
The Request Comment comes OOTB. You don't need to add it.
While performing action on request you can add comment.
Check drop down on request page.
And to add this information you need to have custom adapter which will fetch the comment and add it in email body and then send it to required email id.
Use above mentioned API to get comments.

Those comments won't be mandatory (it's optional)
Anyone can add comments there (Beneficiary/Approver/Requester), how will you find out which one is Rejection Comments.
To avoid such issues, some people have done UI Customization for storing Rejection Comments in different format and to make those comments mandatory.
Thanks a lot for your support , but can any one tell in which form do i need to do changes to make it mandatory while rejection.
There's not any form. You need build your UI/custom action class to make it mandatory. You must know about OIM 10g Flow, Struts etc. It won't be an easy task. If you are comfortable then only proceed.
I have done with reason and email but making field mandatory with OOTB is challenge. If you guys have something please share.
I don't think that you can achieve this OOTB. About customization I have already given you pointers.
