Not like OSSO10g, for OAM11gR1 there is no the out of the box to accommodate the "FallBack" to username/password auth.
You need to make the customization yourself.
Any docs that at least outline how to do that?

In the link it talks about having 2 different resources being protected -- one with x509 and the other with LDAP. We have the situation where we have one protected resource whose AuthScheme is x509, but two different users where one has a cert and the other does not (and will not).
In the doc (second paragraph):
It should be noted that this solution is not for the use case where the user's authentication is rejected due to an invalid certificate by OAM and then the user needs to fallback to a FORM for another authentication attempt.
Is a user not having a cert the same as a user's cert being rejected for being invalid?
No. I think he meant that as a user prompted with the cert "Accept" or "Cancel", if the user selected "Cancel" then the customization would be applied for, However if the user select "Accept", but the user had a bad cert or no cert, that custom would not be applied.
