It appears that theme 24 was previously added.
Specific information as to version number, what you have done previously, and specifically how you are doing whatever you are doing might help.As stated, this is occurring on apex.oracle.com so the version is implied. I suspect this is connected with the APEX 4.2 upgrade applied in Q4 last year (although I have been apex.oracle.com: Performance problems/no response when switching themes). All I'm doing is using the standard Create Theme wizard.
As this is happening on apex.oracle.com it needs the attention of someone from the Oracle APEX team.
I'm not sure how you ended up in this situation, but you had one template associated with theme ID 24 with a name of 'DIV Region with ID'. This was in the absence of theme 24 being installed in your application.
I'll review the data integrity rules how this was even permitted in the first place. But for your specific application, I deleted the one errant row and you can now install Theme 24 into that application.
