just to know what you mean. Did you want to upgrade or to patch? Your ORACLE HOME seems to be 10G databases and your patch is 11.1?
In case you mean a upgrade. Then you can install 11.1 as new clean home on your server and upgrade instance for instance for example with the dbua.
As noted, you are not patching, you are upgrading. You will need to install 11gR1 (184.108.40.206) binaries into a new ORACLE_HOME, apply the 220.127.116.11 patch, and then follow the steps in the Upgrade Guide for 11gR1 to upgrade needed databases to 18.104.22.168.
If you are going to upgrade, you should be upgrading to 11gR2, since 11gR1 is almost desupported
Which patch are you referring to ? The database patching tool (opatch) generally applies any database patch to an ORACLE_HOME. You will need to follow the instructions in the README of the patch. If you need to only patch one instance, then you will have to create a dedicated ORACLE_HOME for each database (i.e they will not share database executables). If you share ORACLE_HOME amongst multiple instances, then you will have to patch all of them simultaneously.
