Edit the form and select the Layout tab. Select your row member and look to the Grid Properties on the right-hand side of the page. Select both "Member name" and "Alias". Save and review.

Thanks for your reply. This is not exactly what I need. When I follow your instructions I have member and alias in one Excel column in SmartView. I would like to have it in two separate columns.
I don't believe the tool supports this.
Just like Jake writes, it is not possible to have name and alias from the same dimension in two row headers, but you can set the form to display name and alias in the same row header.
To do this (188.8.131.52), you just select the dimension and mark 'Name' and 'Alias' in the right panel that appears.
