Check whether you have configured Request Type in your approval policy properly for operational level approval. In the Rule Components section check whether you have configured everything correctly. Also dont raise the request from system admin login as it will be treated as a direct provisioning request and your approval policies will not be invoked. Login through an end-user and test it

To answer your questions:
1. At both request level and operational level the approval policy rule is same i.e. Request Type = Assign Roles and at request level approval is triggering, so i think its not the problem with the Rule
2. I am raising the request through code and not through UI.
Can you check once raising a request through UI with request level auto approval configured and attaching your composite to the operational level policy.
yes it is working as expected and going to the operational level when raised through OIM Catalog.
